By proceeding with this appointment with Trust Tele- Rehab you have agreed to the below Consent:
Remote Video or Telephone Assessment :
- You will be asked questions regarding your physical well-being and how your social and physical situation is affected.
- You may be asked to demonstrate some movements and activities. You can choose to dress as you feel appropriate in order to assist in any demonstration of physical activities.
- Any advice provided by the professional has been accompanied by an explanation of any risks or potential side effects, if appropriate.
- You can ask any questions to clarify information and seek a full understanding of any information and advice provided to you by the professional.
- For video appointments, you consent to the professional being able to view you and any others accompanying you, in your surroundings by video.
- Any information discussed and recorded is done so in accordance with our Privacy Policy (LINK)
- You do not have the permission of the professional to record this assessment. If you wish to record this assessment you must apply by email to our Administration Dept (admin@Trust-telerehab.com) and wait to receive a confirmation email with the agreement of the professional.
